Remote Therapeutic Monitoring
Advanced Care. Delivered Beyond the Clinic.
Recovery and rehabilitation do not stop when you leave our office. Remote Therapeutic Monitoring (RTM) is a clinically validated service that allows our physical therapists to track your progress, monitor your adherence to your home exercise program, and respond to changes in your condition all between in-person visits.
RTM extends the reach of professional care into your daily life, ensuring that your recovery receives the attention it requires at every stage, not just during scheduled appointments.
What Is Remote Therapeutic Monitoring?
Remote Therapeutic Monitoring is a structured, technology-assisted program through which your physical therapist collects and reviews data related to your musculoskeletal or respiratory condition outside of the clinical setting. Unlike general telehealth visits, RTM is specifically designed to monitor therapeutic adherence and patient-reported data on an ongoing basis throughout your plan of care.
Through a secure digital platform, your therapist receives real-time information about your exercise performance, pain levels, functional status, and any changes in your symptoms. This continuous data stream allows your care team to make informed, timely adjustments to your treatment program without requiring an additional office visit for every modification.
RTM is a billable, insurance-recognized service established under guidelines set by the Centers for Medicare and Medicaid Services (CMS) and increasingly covered by commercial insurers.
How It Works
Enrollment in our RTM program is straightforward and requires no specialized technical knowledge on your part.
Step 1 - Enrollment
Following your evaluation or at an appropriate point in your care plan, your therapist will enroll you in the RTM program and introduce you to the digital platform used to collect your data.Step 2 - Home Exercise & Data Collection
You perform your prescribed home exercise program using the platform, which records your activity and prompts you to report on pain levels, functional changes, and any concerns between visits. The process is designed to be simple and minimally time-consuming.Step 3 - Ongoing Monitoring & Clinical Review
Your physical therapist reviews your submitted data regularly a minimum of once every 30 days as required by RTM guidelines and responds to reported changes in your condition. If your data indicates a concern or an opportunity to advance your program, your therapist will reach out directly.Step 4 - Program Adjustments
Based on the data collected, your therapist may modify your home exercise program, update your goals, or schedule an in-person visit if clinically indicated. This responsive approach ensures your rehabilitation remains on track between appointments.
Benefits of Remote Therapeutic Monitoring
RTM offers meaningful clinical and practical advantages for patients managing musculoskeletal conditions, recovering from injury, or progressing through post-operative rehabilitation.
Continuity of Care
Your recovery is actively monitored between visits, reducing the risk of setbacks going undetected and ensuring your program evolves with your progress.
Improved Outcomes
Patients who adhere consistently to their home exercise programs demonstrate better functional outcomes. RTM provides structured accountability that supports adherence and keeps patients engaged in their rehabilitation.
Earlier Identification of Complications
By tracking patient-reported data in real time, your therapist can identify warning signs such as increased pain, swelling, or declining function before they escalate, allowing for prompt clinical intervention.
Reduced Unnecessary Visits
RTM allows minor adjustments to be made remotely, reducing the need for additional in-person appointments for issues that can be addressed through program modification and direct communication.
Direct Access to Your Care Team
The platform facilitates secure, documented communication between you and your therapist, ensuring that questions and concerns are addressed promptly and professionally.
Who Qualifies for Remote Therapeutic Monitoring
RTM is appropriate for a broad range of patients currently under an active physical therapy plan of care. You may be a candidate for RTM if you are:
Recovering from orthopedic surgery or a musculoskeletal injury
Managing chronic conditions such as osteoarthritis, chronic low back pain, or degenerative joint condition
Participating in a home exercise program as part of your physical therapy treatment
Experiencing difficulty attending frequent in-person appointments due to work, transportation, or scheduling constraints
Seeking additional clinical oversight and accountability during your recovery
Your physical therapist will assess whether RTM is appropriate for your specific condition and plan of care during your evaluation or at any point in your treatment.
Frequently Asked Question (FAQ)
No. RTM and telehealth are distinct services. A telehealth visit is a real-time, scheduled video or phone appointment with your therapist. RTM is an ongoing monitoring service that collects data from your home exercise activity and patient-reported outcomes between visits. The two services can complement each other within the same plan of care.
RTM is typically accessed through a smartphone application or web-based platform. A smartphone, tablet, or computer with internet access is generally sufficient. No specialized medical devices or wearables are required for standard musculoskeletal RTM. Your therapist will walk you through the setup process at enrollment.
The time commitment is minimal. Logging your exercise sessions and reporting your symptoms typically takes only a few minutes per day. The platform is designed to integrate naturally into your existing home exercise routine.
Yes. RTM requires documented clinical review of patient-reported data by a licensed clinician. Your therapist is responsible for monitoring your submissions, responding to reported concerns, and initiating contact when clinically indicated. This is not an automated system your data is reviewed by your care team.
RTM is a covered service under Medicare and is increasingly recognized by commercial insurance plans. Coverage and cost-sharing vary by plan. Our administrative team will verify your benefits prior to enrollment and review any out-of-pocket costs applicable to you in advance.
If your reported data indicates a clinical concern such as a significant increase in pain, a functional decline, or a potential complication your therapist will contact you directly. Depending on the nature of the issue, they may adjust your home program, schedule an in-person visit, or recommend follow-up with your physician.
Yes. Participation in the RTM program is voluntary. If you prefer not to continue, you may discontinue at any time. Your standard in-person physical therapy will not be affected by this decision.
